If you are planning on making an appointment for an evaluation for headaches/TMD click on the following link to print out the necessary questionnaire. Once completed please scan and email the completed forms back to polly@drcraiglandry.com or fax it back to Polly at 337-984-2763. Your help is greatly appreciated.
If you are filling out the General Patient Form, please email it to allison@drcraiglandry.com. You can also print it and fax it to Toni at 337-984-2763.
